Leonard Paul Kostyak, Sr., passed away on Tuesday, October 28, 2014. Service: 1 p.m. Wednesday, November 12, at the Dallas-Fort Worth National Cemetery, Dallas. Memorials: Should friends desire, memorials may be given to Community Hospice of Texas, 6100 Western Place, Suite 105, Fort Worth, TX 76107, put on the check or add a note with the check that the donation should go to the Hospice at 1111 Summit Ave., Fort Worth in memory of Leonard Kostyak or St. Andrews Catholic Parish, 3312 Dryden Rd, Fort Worth, TX 76109, In memory of Leonard Kostyak. Leonard was born on October 31, 1937 in Pittsburgh, PA to Rudy and Catherine Kostyak. He enjoyed fishing, watching his Pittsburgh Steelers and Gambling at Winstar. Leonard will be greatly missed by all who knew him. Survivors: Wife, Vicki Kostyak; sons, Leonard Kostyak, Jr. and wife Carrie, and Philip Kostyak and Sara; daughter, Kimberly Stark and husband Alan; grandchildren, David, Trey, Jeremy, Bradley, Colton, Cody, Morgan and Chelsea; great-grandson, Ridge; sister, Emma Walters; mother-in-law, Pauline Hudson and Paul Koopman; sister-in-law, Pat Blackmon and husband Roy; and numerous nieces, nephews and extended family.
